A condition report and a repair request serve different purposes; make sure a problem that needs action is also reported as a request.
Describe a repair so it can be assessed
Include your address, the affected room or equipment, when the problem started and whether it is changing. Mention visible water, unusual odors or loss of service. Photos can help when they can be taken safely. Give current contact details and explain any access considerations.
